Could be an AI error. First thing you need to do is eliminate that possibility. Use the AI tester to test your AI for the scenarios in which your team is calling the bad plays. If the sim isn't finding any matches in your AI, that means it's switching to Default mode for that play, and all sorts of crazy shit can happen then.
I'd also check your Tactics Calendar and make sure the sim used the correct AI for the games in question.
