I've heard people say play a medium zone, I've also heard say stack against the run with FS and SS always spying, what has worked for you?

If it's just a rushing QB, Tagged preset is the way to go apparently.
Going to move this over to FAQ, you'll likely get more responses there too.

The old plan was to go with 100% pass/100% zone/100% short, which doesn't really make sense but it does get your defense moving outside towards the LOS.
This season I've used the Tagged preset a couple times and it's worked pretty well. The tags should look like this:
Power (pHB) = running back that typically only runs between the tackles.
Blocker (bFB or bTE) = back or TE that typically is only in to block, not run routes.
Receiver (rHB, rFB, or rTE) = back or TE that typically is only in to run routes, not block.
Rusher (rQB) = rushing QB.
Speedy (sHB) = running back that typically only runs outside the tackles and is not called on often to run routes.

I did something similar to Northstar. I went 60% Run / 90% Zone / 80% Short.... I didn't stop him, but I slowed him way down.
Conversely, I ran another game with 90% M2M - QB Gashed us for 230+ - and prior to that game he wasn't a threat.
I don't have any experience with tagging. It makes me nervous in the event a tagged HB and the rQB are in at the same time.

I always ran a 3-4
100 medium, 100 zone, 100 balanced
Set spies to 2 max
Set LILB,RILB, MLB, SS to 100%
This splits the 2 max spies among 4 LB sets and 1-3 LB sets (adding the SS to the 1-3 LB sets).

if the team relies heavily on that one QB to run the offense, i always go with the tagged preset. just tag the QB as a rusher and let the sim do the rest. held the best offense in the league to 70 total yards a few days ago. sometimes it works to just keep it simple, but as you can see there are many other ways to do it
