For your specific case, your HB with 2 carries played FB all game. He was blocking for the others. Fix your depth chart. With no player in the TE depth chart slot, it drew the FB to fill it.
A few things that can also trigger play split issue.
- Special teams play. STs eat up a lot of energy. If only 1 back is on STs, then he's going to burn through his energy there. Since most teams are utilizing a high energy rotation split, he's not going to have many plays at HB.
- Custom slots. If your AI is using custom slots, then that's going to dictate which RB is in. As a recommendation, give each of your RBs their own depth chart slot (one in the generic RB, one in RB1, and one in RB2). Then, from your AI, control the play splits with the custom slots (your output will specify which HB is in).The idea of putting each RB in his own depth chart position seems pretty logical and is exactly what I was looking for.

So if the play is setup to use RB, then only that HB will be used. If the play is setup to use RB1, only that HB.. etc.
Two of your HBs are Power Backs so it's easy to put both of them in the RB slot on the Depth Chart with a decent sub ratio. As for your Elusive Back, you may want him in the RB1 spot for outside pitches. If you're not sure where to find this in your OAI, it's located in your play Output...

Romo is right, I changed it right after I read his post without responding here, prior to that I had all three HBs in the generic slot. Now I assigned each output seperately.
Edit: I actually like your idea of putting the two pHBs in the same slot even better so I don't have to worry about splitting the carries evenly.

Any thoughts about my second question? From the percentage settings in my outputs, there should have been quite a few more run than pass plays.
