I only have one example right now but I will edit later with more. If you pause this right when the returner catches the ball you will see 3 wedge blockers and 3 opposing players pretty much in line with one another. Pause again when the returner reaches the 15 yard line and you'll see that the 3 wedge blockers all converged on the same target. Something is very wrong about this.

The second example isn't as bad but the wedge blocker on the left of the screen targets the player to the far left instead of converging down on the more immediate targets.

Third example we have 2 guys coming to the left and one to the right. The blocker in the middle should obviously grab on of the guys on the left, leaving the right guy for the right wedge blocker. Instead he moves way out of position, leaving the left wedge blocker to block two guys.

yeah, the wedge targeting has been terrible the past few seasons. most of the time, its the bw2 that goes after the wrong guy and that either causes an unnecessary double team, or the outside bw doesnt know what to do at all. i thought it was a vision thing, but even with low vision, the bw2 goes haywire.

Here's another example late in a game, middle guy goes up and gets his guy but the other two kind of clusterf*ck around and decide to switch or something. One ends up not blocking anybody and the other gets the guy on the opposite side of him.

It comes down to lane assignments and vision checks (which substitutes for Discipline in this game). I would expect failures like this to happen at the lowest levels much more often than higher up. I've always adjusted these issues by varying the speeds and agilities of the wedge blockers so that they aren't all the same (or very close to the same). This causes the quicker and faster blockers to pick up the first guy through... and, once he's engaged in his block, the other wedge blockers usually look for others to block (but not always).
Having better lane discipline would help a lot... and better "assignment" blocking would too. (To explain what I mean; A blocker should have an assignment to block a certain dot [usually the one in his lane] that can be over-ridden by things like faster dots "in his lane/zone" that are NOT his assigned target, override his assignment and he blocks them instead as they would be the closer threat [read "vision check"].)
I think this mostly agrees with the OP and therefore I +1 this thing.

Yeah I thought it might have been a vision issue too. The vision of the wedge blockers I've been posting replays to seem like they should be high enough to not mess up this often. I'll check some WL games as well to see if there are issues there. This team seems to have it happen at least 75% of KR's.

Okay so there's all the WL footage I can go through right now. It looks to me like the issue is all 3 players are going for the most immediate threat with no knowledge or thought* to who their teammates are going to be blocking. Seems like what needs to happen is each blocker sticks together and takes the most immediate threat in their blocking direction. So if there's an immediate threat to the far right the right wedge blocker will take him and the left wedge blocker will not worry himself with it.

seems like lately, instead of each wedge blocker picking up a player, i'm seeing way more double teams allowing someone a free shot at the kr.

like in this example, why is the bw2 looping behind the bw1 when no one is ner them yet?

Yeah I've seen that same behavior. Special teams needs such a looking at. There are 3 or 4 suggestions threads with 4k+ upvotes that aren't even in the works. Special teams is 1/3 of the game and it seems like in this sim it's been ignored for too long.

Not a TD? That play is depressing though because if the wedge guys blocked left like they should and the returner followed his blockers there's another 20 yards to be had.
