What causes Thrown Low's? I thought Strength would be most of it. When I fed Strength, it appeared he threw more?

I notice that throwing with the bullet passing style results in more low throws. I'd guess that throwing with bullet and low strength would be a bad combo to have.

I am using pretty high loft and my strength on the last game was 49 with throwing near 105.

The better question would be why is there no "thrown high" or "thrown wide"? Seems like it's low or right on.

49 strength is still very low, I wouldn't expect that to fix the problem if strength was the problem in the first place.

Bad routes and pressure contribute. I think its like a bad pass but other factors contribute so it doesnt give you the error.

Actually, that's not a good question at all. "Thrown wide" happens all the time, that's when the ball isn't on line with where the receiver is. "Thrown high" is simply when the receiver can't jump high enough to grab it, or jumps high enough to get a hand on it and tip it.
