Team with the best record I assume, and if they had the same record I assume points scored would tie break, unless by chance the two teams played.

Nope. But I've suggested before that it should be a 2 point morale/chemistry boost or something.

Should be played at a neutral site imo just like NFL...
Either give both teams equal share of revenue or no revenue bonus as there's really no way to say which team deserves it more than the other since the two leagues are impossible to accurately compare with each other (except for the one sole out of conference game at the end of the season).
Unless the two teams just happened to play in that last game
