What happened? Looks like the MLB released the TE, who the SS picked up. But the CB released the WR to the SS, who was occupied with the TE already?

I think that was just a matter of the players got fooled. Just a GLB thing with the sim. In otherwords, it probaly couldn't of been avoided.

Both the Safeties went into a zone. Just the play went towards the SS more. Looks like Cover 2 to me.

TBH, if you're playing a 4-2-5 Cover 2, you're practically begging for your defense to get ripped to shreds.

It is not a cover 2. The FS lines up deep center with the ss set off to the strong side. That is probably a cover 1 with pass or balanced focus. The LOLB is blitzed through the center of the line which means that the LB shift was weak side. The HB wasn't running a route so the MLB was covering the TE which is next in his progression when in man coverage. The SS was in man coverage. When the ball was thrown, the MLB made his vision check and chased down the pass. The SS did not make his vision check and continued to chase the TE along his route.

I guess this play was intended to defend against the pass and the Weak side pitch.
