I've been on a couple different teams that have a couple of things in common. Lots of Sacks and One reciever getting a grossly disproportional amount of the passes. The obvious problem could be that the QB is favoring that reciever, but in both cases that is not the case. What else in QB tactics of the teams' offensive tactics could cause this?

That doesnt appear to be the case...our #2 reciever had a theoretically better match up and got 1 or 2 passes....

Low QB vision might; the QB might not be checking down to his alternate reads quickly enough.

The QB has vision at about 57, strength about 56, and throwing in the mid 80s...

It's this...but the "outmatching" depends on the sim's perspective, not necessarily on what you might think is a mismatch. That particular equation is a little counter-intuitive in my experience. Also, it is more of a question of "best match" rather than "outmatch." You could be over-matched at every spot, but if one matchup is calculated as greatly better than the rest, the targets will still end up there with high frequency.
You can actually learn a lot from observing this particular phenomenon

I watched most of the plays involved....it still seemed like the QB was "forcing" it in to that one guy when other guys were more open. I'm going to check some things out this next game and see what I can learn...
