You just pick it, give the "Output" a name and choose the "Percent Chance" and your done? You do not mess with any of the other options correct??

Yes. The more info that you put into the "output" makes your play more and more specific.
Something to be careful about is to not set too few extremely limited plays since those become really easy to scout out.

I think am getting the hang of this. Thanks guys!

Its probably good to design your own plays and mix them with the Pre-Designed ones right?

That's what I do, because the not all the pre-designed plays will work for your team and your "system". Plus, there isn't a 5-wide defensive formation, only 4-wide, and those don't work well when the offense runs 5-wide (in general).
