Are plays like this, where the QB ignores the wide-open slot receiver and instead forces it to a covered WR1 a vision issue or something else? http://goallineblitz.com/game/replay.pl?game_id=152598&pbp_id=6332525

The QB in question has 54 vision, and I didn't really notice any pressure on that play. The same thing happened a bunch of times last game, especially against cover-1 with the slot receiver open and not getting looked at.

There isn't any wide open receiver in this play.
Pause it and fast forward frame by frame and you'll see that the CB breaks away on the throw because he's going toward the receiving WR for a tackle. He simply has good vision. I've seen this in maybe.. 50 plays?

It's a medium distance pass so the QB is waiting for a medium route to complete, WR1 does it first.
