LOLB: Run FOCUS, Combo Pass Rush, wrap-up . Close/Aggressive Coverage.
MLBs: Run defense,Combo Pass Rush, focus, wrap-up.Close/Aggressive Coverage.
CBs: PASS defense,Combo Pass Rush, focus, wrap-up. Medium/Medium Coverage.
FSs: Pass defense, Combo Pass Rush,focus, wrap-up. Medium/Medium Coverage.
SSs: Pass Defense focus,Combo Pass Rush, wrap-up. Medium/Medium Coverage

There are other factors than just the players' settings.

How their builds match up to the opposition along with the defensive play calling make a huge difference as well.

I quoted part of your problem, but also what Madlister said is very true.

Medium coverage distance on your CBs will never stop the pass. It can stop the deep bomb stuff and limit the damage if you're badly outclassed (you'll lose by 15 instead of by 50), but medium passes will destroy you.

I don't like putting the SS in Medium either, because he's often covering the TE.
