Is this solely the Lead Block SP and Blocking skill?
regardless, shouldnt the FB at least head in the same direction as the pitch play?

It's the design of the play, FB blocks in the middle.

but its a pitch outside, why wouldnt the FB try to block someplace the HB is headed instead of someplace he isnt?

is there a way to get a play designed where the fullback leads the way?

It's how the play is designed. There's nothing wrong with how the play is designed. Also, it's got nothing to do with why the play failed. That is a very good play, if the defense doesn't overload to stop it, which happens quite often.

Actually, there is something wrong with how it's designed. The FB should seal off the hole left by a pulling guard, not go double the DT with the LG (complete waste).

Either the LG pulls while the FB seals the DT (for as long as he can) OR the FB should seal the hole left by the pulling RG.

Right now, the RG pulls, the FB doubles the DT, and there's a gigantic hole for a speedy LB to shoot through and blow up the play.

This is why fast LB are so good. You hit the holes that the AI leaves.
