Fail? Your player is 366 days old and only level 45. As a reference, my level 59 FS is 348 days old, and I could have had another level or two had I boosted 'correctly' his first season (i.e. waited until the end of season vs. boosting early in the season). You either weren't boosting 3 levels a season or weren't getting enough playing time to get max XP. Looks like your guy took all of season 7 off, which hurt. Unfortunately, he will never get the build to be able to compete at a high level before he starts declining. I'd retire him and start over with a better approach, or be content playing him in low level leagues until he reaches retirement due to age.
