The difference is that at HB and WR there are usually role players who do certain things quite well to take over a particular role easily for a play or 2. QB is really not such a position. Generally speaking you need the QB to be able to take on several roles just in case something occurs. Further, the role players can just be used as a connection where as a QB will have to touch the ball. Sure he may just be able to hand the ball off in some cases, but if he is in for an entire series or something of that nature he will probably have to throw the ball at some point. In general, if the back up QB was who you wanted throwing the ball he would be your starter. Also, even if the backup QB is there just to hand the ball off there is several exchanges that change slightly which could cause issue. Generally speaking, you don't want any of those exchanges changing unless they absolutely have to. Therefore, it is not a great thing if a QB is missing plays. Granted, he comes back in, but it is still concerning when he comes off the field because of pain, and this is an absolute truth for every starting QB in the NFL, and yes it is more concerning for a QB to come off than a HB or WR.
