OP Updated to include a drop down option.
Example:
Player Name Options:
1) I don't care about this player's name. Rename him to "Retired Player" so anyone can reuse the name.
2) I want this player's name to stay reserved and live on forever.
3) Re-create this player.
or use a checkbox option to re-create the player.Both would work
