First and formost, you guys are going to tell me to get vision up, but my vision is at 52 without equipment. The past two games my team lost twice against two teams we could beat. We think we got simmed, but I think its because of my QB.

Last two games, I threw 6 INT's and go sacked 6 times.

I gonna have to say its b/c of Borts new changes, even though he said there shouldn't be an affect with 50+ vision.

QB's with decent vision hardly ever miss an open man on a checkdown, which means they will ALWAYS take advantage of the situation. I'm making them slightly more likely to miss the open guy unless they have really high vision.

No, he stated that FORMERLY QBs with 50+ vision almost never made a mistake. The change was he made it so that even with 100 vision you still have a chance to make mistakes. The change is specifically for high vision players.

From the post in Announcements:

QB's with good vision (50+) hardly ever miss an open man on a checkdown, which means they will ALWAYS take advantage of the situation and basically never make a mistake at all. I'm making them slightly more likely to miss the open guy, since everybody's human even if you have 100 vision.

To answer the original question:

Late pass, the WR was open for a second but the DB jumped the route. The WR on the right gets open right as the ball is thrown, just a little too late.

In my opinion this is likely to be due to Bort's change as the HB was wide open and not thrown to.

Bad throw, the WR was open and had his man beat but the ball was under thrown.

Again single coverage and your WR gets behind his man but the ball is picked before it gets there.

WR was open, the ball was over thrown or the receiver can't jump well. Or both.

No one got very open here, the #1 was closest though he wasn't passed to. Could be another bad decision but probably just an example of the defense having the right play called.

52 vision is pretty good, the culprit could be low strength, throwing or tight spiral since at least half the passes were off target. But without knowing those stats there is not much to go on when looking for a solution. Don't forget though that if your receivers do a better job of getting open then the QB has an easier job making good reads, but again a lot of those passes were to open receivers but weren't on target.

Throwing would be my focus until you get it to 70+ then soft cap strength. Throwing will improve accuracy and strengths will add velocity to your passes so you can hit speedy receivers in stride and give the defense less time to react to the ball while it is in the air.

Edit: I sent you a PM with some more info.
