Lot of battle rolls in this game.

Make tackle vs break tackle
avoid fake vs fake

Keep in mind both sides will have a "battle score" of attributes, SAs, VAs, and fake% as a boost to the total. So giving a dot with a LOW base fake score and LOW SA's, +22.5% fake% probably won't do much. But if you're making a dot where fakes are a priority, it's a must.

Well essentially, in terms of watching replays, you can think of a fake as causing the defender to delay. They just sort of stop for a split second.

Head Fake, Juke, Catch Fake and Pump Fake by the QB.

Anyone who runs a route can attempt to use fakes to get open, if the route calls for them.

Any ball carrier set to elusive running will attempt to use fakes to keep the tackler out of tackling range (including pass catchers after they have made the catch, but that's rare because the defender is usually close by already).

The QB may attempt to use a Pump Fake to hold a defender in place, trying to get his receiver open. These get more and more rare as the dots age. It'll flash quickly on the defender, like the FS here.

The % chance is a little bit of a misnomer. It's a multiplier to your chance to do something, in this case Fake. These numbers are just for easy math, no one knows the scale involved.

If your battle score indicates you have a 20 out of 100 chance to score a Fake (20% chance) and to that you add 10% Fake chance gear, your 20 would increase to 22% not 30%.

If you start with a 50% chance to get a fake, and you add the 10% to that, it would be 55% not 60%.

That's why % chance gets more important as your base chance goes up. Even a huge modifier on a low base score won't do much of anything.
