How does it trigger and when does it reset?

Someone with a ton more time than I could find this in the old Q&A's cuz I know it got asked there. I do know it accumulates in it's penalty (aka, the more ya spam the play, the more penalty). I don't think it resets in a game but I also don't think it carries from game to game. Also, consider, there's gotta be a counter balance because the basic AI will run certain plays MORE if they're successful and less if not so... it can't penalize itself for calling it's own plays more... I would think.

basically does not exist
if you ran one play 100% of the time, it's doubtful you'd notice a difference between the 1st and last time
so if you run 4 different plays, you'll never see it

In my rookie league game I basically ran 1 play the whole time
Against 1 defense response and it did much worse after first few drives
Could be stamina and moral just hit offense harder

So the difference between the first 4 times you ran it and the last 4 times, is like less than 1 yard per attempt?
Could that be your o-line getting tired?
Could that be a morale issue?
even if the alleged "repeat play penalty" is the issue, that's a crazy huge penalty there... maybe 1 whole YPC after an entire game.

there's about 20 varieties of slams, blasts and dives alone. Build a playbook out of all of them, and I'll bet you'll see at least a 1 YPC drop between the 1st and 4th quarters
