I've noticed in the past seasons that some playoff teams get home nachos and some don't - do you have to spend promo money throughout season in order to get playoff nachos?

You have to spend the money on the nachos im pretty sure. Some teams just don't do it.

I've never had to manually add nachos for home playoff games. They just appear.

nacho's are given for teams in pro leagues (regional pro on up) for home games.

Below that home team gets ticket sales.

Both get concessions.

Also, home teams can buy Nachos in the leagues below Reg Pro, so if you want to buy a promo on top of your ticket sales, go for it.
