I am the defensive coordinator and do not have any type of set up with the CB being switched out or anything at all. My starting CB played 0 plays the previous game and then 4 this past game. Can anyone tell me what could be the problem?

It looks like you are just misunderstanding the role of the slot specific depth chart slots.
Here's how it works:
The specific slots take priority over the generic. What the generic CB depth chart slot does is fill the empty slot specific slots. So having iCanCover in the generic slot, but not the specific slots means he only comes in as a 'fill'. As it stands for every play, Ronnie Hadley will be CB1 and Joachim "Champ" will be CB2.
I'd recommend only using the generic CB slot given the low levels.

You have found it for me.
I did not do that so it seems like it was the previous DC that did that. Now it makes perfect sense. It might of also been the owner of the team. Some owners just don't know how to own. Asking people to DC and OC jobs then he wants to run the team like he's Jerry Jones. I started as OC, then the DC had 2 bad games when working on his play calling and the owner removed him, told me to do defense after I had spend hours setting up offense, and then he took over offense and put in a standard Big O preset for the offense with no custom plays of no offensive AI. Now I can see how owners can ruin their own teams.
