who faked him, the ball? the CB makes a break on the ball and then just kinda sits there while it goes over his head.

Most likely a head fake. If you watch the play frame by frame, you'll see the CB cut off one direction and the WR go the other direction, then the CB just stops. Anytime that happens on the PbP, it's some sort of vision-check fail caused by a fake.

the problems i see are this:

1. he breaks the opposite direction as receiver goes. usually when i see a guy bite on a fake he just freezes
2. he breaks after the ball is in the air
3. he breaks perfectly into the path of the ball. i don't think that is a coincidence

i think what is happening there is that the CB is trying to make a play on the ball but misjudges is horribly. the thing is, he sits there so long and has sufficient vision/SV that he should have corrected or made a motion to correct. he sits there for 10 solid ticks doing nothing while his guy sets up in teh back of the endzone. then, when the ball passes immediately overhead, he can't PD or pick it.

the issue with that is he is set to medium. is there a vision check that the CB has to pass to know whether he has help over the top? seems he should know he doesn't based on the play call.

That was just a head fake, but honestly you shouldn't be able to do them to people behind you.

This game has had problems with coverage in the endzone since season 1
