Obviously, I don't know your build, but here are a couple of things you can do.
#1 - don't blitz your strong safety, have him positioned inside and double cover the #5 WR or have him play that short zone.
#2, have one of your DT's drop back into that zone to stop that play and use your SS to cover somewhere else or blitz from the far outside. Having your SS go up the middle on a 5WR play when rushing 4 gets you no where.
#3 - use the custom D creator and spread your D Line as far to the outside as you can making the O line widen their blocking schemes and line up your #5 CB on the inside of the opposing CB, so you are already on the inside when the play starts.
