I've tried setting all the rushes in my AI to HB only, I've tried removing all the QB/FB rushes from the playbook (but for some reason we have to keep one) and yet every once in a while the QB and FB will sneak in a rush play and it is starting to annoy the hell out of me.

Any idea how to stop this (if it is at all possible)?

Set the basic Al to 0% for both, but other than that, there's not much you can do.

You're missing something in one of your outputs.

I've gone over my outputs a dozen times, and all the rush plays have HB set as the main rusher. I've also set the AI to 0% for both. Yet every once in a while they sneak in a FB rush and its driving me nutty.

It's something in your red zone plays. Your FB has two runs for a total of 1 yd and 2TD's in your last 2 games.

My guess its not the outputs, but the inputs. You've got a hole. And when it can't find an output, it defaults to the basic and runs a random play.

Look at one of your plays that the FB ran. Put that exact situation into the Off AI tester and see if a play is spit out. Or if it says no input.

If it does give an input and there are no FB run outputs, for that input, then its the AI being quirky.

I can watch 99% of my plays and know the exact input/output combination being played. But I have had about 2-3 plays in my 5 games this year, that I just scratched my head and didn't know where the heck it came from.

This helped a ton. I did have a hole in my inputs, and it was a pretty stupid hole in my first down AI. Thanks a bunch, I'm pretty sure I fixed it.
